Once you become a 4THEEBOC TV Network subscriber—$10.00 monthly or $120.00 annually—remember to register the church of your choice, preferably the church where you are a member in good standing. Complete this form to the best of your ability. If you have a business card, bulletin, or any material with the church or pastor’s contact information, feel free to submit it along with this form.
How Your Giving Blesses the Church of Your Choice:
1. The church must have a minimum of 25 paid subscribers.
2. Each subscriber must remain in good standing for at least six months.
3. Each subscriber must complete the form to ensure the church qualifies.
Once your church reaches the 25 required subscriptions, it will qualify to receive the 4THEEBOC Give Back Program—benefiting both the House of God and the First Family. When this happens, both you and your church will be notified.
